I, the undersigned
(1) Declare that I have full parental authority over and bear full responsibility for the Minor and can therefore validly give this consent. I confirm that all information given on this form is accurate and am duly aware that I could be held liable if I make any false statements.
(2) Authorise the company operating the Paris Saint-Germain Academy, i.e L.C.F FOOTBALL TRAINING LLC, a company incorporated in the United Arab Emirates under the Licence Number 750705 whose registered office is at Bur Dubai, Dubai (UAE) (P.O Box: 211906) and Paris Saint-Germain Football, a French company registered under the reference RCS Paris 382 357 721, with registered office at 24, rue du Commandant Guilbaud – 75016 Paris, France and PSG Merchandising, a company incorporated under the laws of France under the number 451 680 458 whose registered office is at 24 rue du Commandant Guilbaud, 75016 Paris, France, hereinafter “PSG Companies”, without any charge, to:
develop, adapt, broadcast and reproduce Mine and/or the Minor’s name, image and/or voice, as recorded in photographs taken and/or video filmed by the Paris Saint-Germain Academy or any third party it appoints for such purposes (“the Content”) at events organised or attended by the Paris Saint-Germain Academy, i.e.: training sessions, matches, tournaments and interviews.
all activities arranged or supervised by the Paris Saint-Germain Academy such as, for example, Summer Camp, Halloween Camp, EÏD, Christmas etc. (“the Events”);
use, together or separately, any part of Mine and/or the Minor’s name, image and/or voice included in the Content, whether reproduced in full or as extracts and live or pre-recorded, in any internal and/or external communications (for educational, informational, promotional and/or advertising purposes) in black or white, without limitation as to volume and on any media now known or later developed, including all paper, audiovisual, electric, electronic or magnetic media, as well as on the Internet and in the press; as long as these operations do not undermine the integrity of their personality. This consent is valid worldwide for a period of five years as from the Content’s first use.
This consent includes the right for the Paris Saint-Germain Academy, and PSG Companies, at their discretion, to add to the Content any written or spoken text, captions, slogans, brand names, distinctive signs or other graphics, or to incorporate the Content in any of the above.
(3) Shall hold harmless the Paris Saint-Germain Academy in the event of any legal proceedings and/or action pursued by any individual or legal entity claiming control over use of Mine and the Minor’s attributes and wishing to oppose their use by the Paris Saint-Germain Academy. personal Shall hold harmless the Paris Saint-Germain Academy in the event of any legal proceedings and/or action pursued by any individual or legal entity claiming control over use of Mine and/or the Minor’s personal attributes and wishing to oppose their use by the Paris Saint-Germain Academy.
(4) Acknowledge and accept that any data or images published online (including on social networks) will remain accessible beyond the five-year period for which I give my consent. I may, however, request that all personal data and pictures/videos of Me and/or the Minor be removed from the Internet by emailing the Paris Saint-Germain Academy at legal@leclubfrance.com
This consent:
is subject to automated processing in accordance with applicable data protection laws. I may request information, access, correction, deletion, opposition, or a copy of my personal data at any time by contacting legal@leclubfrance.com
is governed by the laws of the United Arab Emirates, and any dispute relating to its validity, interpretation or performance shall fall under the exclusive jurisdiction of the Dubai Courts.
(5) Authorise the Minor to:
take part in the youth football training sessions organised by the Paris Saint-Germain Academy his/her age group;
travel as necessary for the Events (train or bus trips between sites);
take part in the various activities proposed in connection with the Events organised or attended by the Paris Saint-Germain Academy.
(6) Declare that I will arrange and pay for the Minor’s travel to the Paris Saint-Germain Academy and that I will remain responsible for the Minor until he/she is met on arrival at the Paris Saint-Germain and Academy training centre.
(7) Accept that the Paris Saint-Germain Academy will be responsible for the Minor as from the time at which he/she is met at the training centre by Paris Saint-Germain Academy staff and until he/she leaves centre. Furthermore, I:
certify that I have been duly informed of the general terms & conditions and rules applicable at the Paris Saint-Germain Academy and that the Minor agrees to abide by them;
vouch that the Minor will also follow any instructions given by Paris Saint-Germain Academy staff;
shall pay for any wilful damage committed by the Minor at any time during the Events.
(8) I confirm that I have arranged all necessary insurance for the Minor in connection with the Events (including public liability, travel, theft, accidents, medical repatriation, etc.). I also undertake to promptly notify the Paris Saint-Germain Academy of any change or issue relating to this insurance.
(9) Certify that the Paris Saint-Germain Academy has:
informed me that it does not provide insurance cover for any injuries the Minor may sustain during the Events; and
recommended that I take out my own insurance to cover any injuries the Minor could sustain through playing football at the Events.
(10) Waive any and all rights of claim against the Paris Saint-Germain Academy and its members. This waiver applies in particular in the event of any accident, injury, theft, damage to personal property or any other incident occurring during the Events.
(11) Expressly authorise the Paris Saint-Germain Academy to take any necessary action and decision regarding the Minor’s medical care in case of an accident, if they are unable to reach any holder of parental authority or legal guardian.
(12) The Paris Saint-Germain Academy shall promptly inform me if the Minor is hospitalised.
(13) Hereby inform the Paris Saint-Germain Academy that I will immediately notify the Academy of:
any allergies, food or medication intolerances, and any required action in case of exposure;
any asthma, medication intolerance, or other medical condition affecting the Minor during physical activity;
any pain, limitation, or difficulty the Minor may experience during certain types of physical exercise.
(14) Accept that the Paris Saint-Germain Academy will take all necessary steps if the Minor fails to comply with basic safety rules or standards of good conduct during the Events, and that I will be financially and legally liable for the Minor’s actions. I will in particular be required to pay for the Minor’s return travel home.
(15) Authorise the Minor to travel in connection with these excursions in any vehicle driven by a Paris Saint-Germain Academy employee (bus or car) or a professional driver (bus).
(16) Undertake not to pursue any legal action against the Paris Saint-Germain Academy or to claim the reimbursement of any expenses incurred in the event that the Minor is sent home for failing to comply standards of good conduct.
(17) Acknowledge and agree that, although the Paris Saint-Germain Academy (operated by L.C.F Football Training LLC, Dubai – UAE) coordinates the Minor’s participation in the PSG Academy World Cup taking place in Paris, it is not the operator, service provider, or legal controller of the facilities, accommodations, transportation services, venues, or third-party suppliers involved in the execution of the Event in France. Accordingly, I acknowledge that:
(17.1) All services occurring in Paris — including but not limited to accommodation, transportation, catering, event facilities, field access, logistics, and security — are operated by independent third-party providers selected by the Paris World Games or by local French service providers. The Paris Saint-Germain Academy shall not be held liable for any act, omission, delay, negligence, error or default committed by such third parties.
(17.2) The Paris Saint-Germain Academy shall not be responsible for any accident, injury, theft, loss, property damage, or incident occurring in France at hotels, restaurants, stadiums, transportation vehicles, public areas, or any location used during the Event.
(17.3) The Paris Saint-Germain Academy shall not be liable for delays, cancellations, strikes, force majeure events, weather conditions, changes in schedules, travel disruptions, operational restrictions, or decisions taken by Paris World Games, local authorities, security personnel, transportation operators, hotels, or any French institutions.
(17.4) The Paris Saint-Germain Academy undertakes to provide reasonable supervision of the Minor during official activities of the Event; however, it cannot be held liable for any misconduct, negligence or action by the Minor outside scheduled activities, any incident occurring if the Minor fails to comply with instructions, safety guidelines, or behavioural rules, or any situation arising when the Minor leaves the group without authorisation.
(17.5) To the fullest extent permitted under UAE law, the liability of the Paris Saint-Germain Academy for any claim related to the Minor’s stay, travel or activities in Paris is strictly limited to cases of proven gross negligence or wilful misconduct directly attributable to the Academy itself, excluding all liability arising from third-party providers or circumstances beyond its control.
(17.6) I acknowledge that I remain fully responsible for the Minor’s behaviour, personal belongings, medical needs, insurance, and travel documents, and that the Paris Saint-Germain Academy does not provide insurance for the Minor’s participation in the Event.
(17.7) I hereby waive any and all rights of claim, and undertake not to seek compensation, reimbursement or damages from the Paris Saint-Germain Academy for any event, loss, injury, damage or incident occurring in Paris that is not directly and exclusively caused by the Academy.
(18) In accordance with applicable data protection regulations in the United Arab Emirates, I acknowledge that the personal data provided in connection with this form will be collected and processed by the Paris Saint-Germain Academy (operated by L.C.F Football Training LLC) solely for the purpose of organising and managing the Events, supervising participants, communicating with parents or legal guardians, and inviting players to matches, tournaments, training sessions, and any related Academy activities.
Personal data may also be collected during the Events in order to communicate about future activities or to ensure the proper functioning and security of the programme. Personal data may be retained for up to six (6) months after the end of the season.
Only authorised PSG Academy staff members may access this data, and appropriate technical and organisational safeguards are implemented to protect confidentiality and prevent unauthorised access, alteration, disclosure, or destruction. Parents, guardians or participants may request access, correction, deletion, restriction, or a copy of their personal data at any time by contacting legal@leclubfrance.com. Proof of identity is required to process any request.
This personal data clause is governed by UAE law, and any dispute concerning its validity, interpretation, or enforcement shall fall under the exclusive jurisdiction of the Dubai Courts.
